What is a Community Health Center?
Community Health Centers are local, non-profit health care centers providing comprehensive primary care to all residents. We serve as patient safety net and maintain an open-door policy, providing high quality affordable care regardless of an individual’s income or insurance coverage. Also known as Federally Qualified Health Centers, the centers are located in areas where care is needed but in short supply.

How do I become a patient?
You may visit any of our providers by calling to make an appointment at 877.374.4991 or fill out a request for appointment on our website.  We will be happy to assist you with transferring your records to CFHC when you make your first appointment, or your clinic’s Patient Service Representative will help you at the time of your first visit.

Are you open at night or on weekends?
Some of our clinics open for services in the evening and weekend. Please call your local health center for specific hours.

Are services free?
No, services at Coastal Family Health Centers are not free. We provide sliding fee discounts to eligible patients based federal poverty level.

Do I have to pay at the time of service or should I expect a bill to be sent to me?
It is our policy that all payments, co-payments and deductibles are due at the time of service. Statements are mailed out on a periodic basis for any outstanding balances owed. Please ask our cashier for further assistance on payment arrangements.

Can I come to Coastal Family Health Center if I have insurance?
Yes, we accept all private/commercial insurance plans to include Medicare and Medicaid.

What do I need to bring to my appointment?
  • Valid Photo ID
  • Current Proof of Income (if applying sliding fee) and residency
  • Insurance card(s), including Medicaid and Medicare at every visit
  • For your children, please bring immunization record
  • List of Current medications
  • Copayment or sliding fee costs
  • Please notify your provider team of any medical care you received outside of CFHC prior to or at your appointment.

My medications are expensive. Can Coastal pharmacy provide any savings?
As our registered patient, you qualify to use our pharmacy which offers medications at reduced costs. Some medications are at a better price than most of the drug stores in the area. In addition, we offer a Prescription Assistance Program for those without prescription coverage who meet the income guidelines. Please contact our Social services for further details.
